I understand that every month on my billing date, I will receive a pre-paid credit on my account as per the membership plan agreement terms.
If I have not redeemed the pre-paid credit within 2 months, the pre-paid credit will expire permanently.
I will not receive any refunds or credits for any unused prepaid services. All membership sales are final.
I am able to give my pre-paid credits to another person if I am unable to use them myself during the 2 month period that the credit is available.
I understand that there is a 3-month minimum commitment with this membership agreement. At least 30 days notice is required for membership cancellation which will result in one, additional, final billing cycle. I will not be charged a cancellation fee.
In order to cancel, I must go online to the provided link when I request it, and follow the instructions. Master Estheticians and front desk staff members do not have the authority to process any membership cancelations or a membership pause. Only administrative staff has the authority to process membership cancelations or a membership pause if I have followed the complete instructions to cancel my membership or to pause it.
I may cancel an appointment with no charge with a minimum of 24 hours notice prior to my appointment.
A late cancelation within 24 hours of my appointment will result in a charge of 50% of the appointment cost.
If I do not communicate through voicemail or text of the need to cancel my scheduled appointment, and I do not come in for my appointment, it is classified as a "no show". A "no show" for my scheduled appointment will result in a full price charge for that visit or a membership credit loss. I may be able to reschedule my appointment for the same day with no charge as long as an appointment is available on the same day for another time. However, if an appointment is not available to reschedule on the same day, it is considered a late cancellation of my appointment and will result in a charge of 50% of the appointment cost.
I authorize Feelgood Fascia to charge my card monthly for the membership fees. Monthly fees will be withdrawn on or after the same day of each month. Additionally, I authorize Feelgood Fascia to charge my credit card on file, for any services received at any time, including appointments that are considered late canceled or which are considered a "no show". This electronic contract and immediate use and benefit of my membership indicates my agreement to be bound by the terms, conditions, rules and regulations of this agreement.
One freeze of up to 2 months is allowed per calendar year for a fee of $35/month. Any unused session credits during the freeze will carry forward.
SECURITY POLICY: Feelgood Fascia's respects our customers’ right to privacy and need for securely stored information. We have appropriate security measures in place in our spa facility to protect against the loss, misuse or alteration of information that we have collected from you. Feelgood Fascia does not share our customer information with third-party vendors.
